BT vs RBT: what a behavior technician needs in Missouri

Last researched October 21, 2026

Behavior technicians are not all regulated the same way. This page separates what the BACB requires, what Missouri and its Medicaid program say, and what remains unverified.

Behavior technicians and the RBT credential

Unregistered BT may deliver services

Only in some situations. See the detail below.

Detail

As of 2026-10-21, MO HealthNet (MHD) guidance allows an unregistered behavior technician (BT) who has completed the 40-hour training and passed the RBT initial competency assessment to deliver services for up to 90 calendar days from the date they pass that assessment. They must then pass the national RBT examination and hold the credential by day 90. This is an MHD participation rule, not a universal requirement for every Missouri BT setting. It also conflicts with the regulation: current 13 CSR 70-98.030(4)(C) still requires technician certification and does not state the grace exception, so treat the grace period as MHD guidance rather than unqualified regulatory permission. Services are billed by the licensed supervisor using the HM modifier plus HO or HN, as documented in the medicaid_aba section.

State-specific requirements
  • MO HealthNet participation requirement: age 18 or older and a high school diploma or national equivalent (MHD ABA grace-period bulletin)
  • MO HealthNet participation requirement: 40 hours of training and a passed RBT initial competency assessment; the 90-calendar-day clock runs from the assessment pass date (MHD guidance)
  • MO HealthNet participation requirement: pass the national RBT examination and hold the credential by day 90 (MHD guidance)
  • Written supervisory relationship documented with a licensed practitioner (older official MHD manual)

What Medicaid says about technicians

Medium confidence: reputable secondary source
Who may deliver services

As of 2026-10-21, per the MO HealthNet Behavioral Health Services Provider Manual (the officially opened version is dated 11/3/2025), MHD reimburses behavior analysts, assistant behavior analysts (including provisional licensees) and psychologists (with ABA specialty approval) for ABA for participants under age 21 with ASD. Technicians provide direct implementation under a licensed supervisor; services are billed by the supervisor using modifier HM together with the appropriate modifier for the supervisor (HO or HN). BTs/RBTs may not enroll as MO HealthNet providers. ABA is paid fee-for-service even for managed-care enrollees. Under 13 CSR 70-98.030(8)(A), payment for assistant services goes to the supervising/employing LBA, and payment for technician services goes to the supervising/employing LBA or psychologist, potentially through their agency. Being eligible to render services does not by itself entitle a person to direct payment.

RBT required for technicians

It depends. See the condition below.

RBT condition detail

Sources conflict. MHD guidance (Bulletin 47-19, effective April 1, 2024) allows behavior technicians (18+, high school diploma or equivalent, 40-hour training done, initial competency assessment passed) to render MO HealthNet ABA services for up to 90 calendar days from the date they pass the RBT initial competency assessment, and they must be BACB-credentialed RBTs by day 90. The MHD hot-tip reminder (published June 26, 2026; this is the reminder's publication date, not the policy's effective date) says services after day 90 without an active BACB credential must stop and claims are subject to full recoupment. However, the current regulation 13 CSR 70-98.030(4)(C) requires RBT certification and has not been shown to include the grace period. Confirm with MHD/MMAC before relying on the grace period.

National baseline

Per BACB: the Registered Behavior Technician (RBT) is 'a paraprofessional certified in behavior analysis' who assists in delivering behavior-analytic services under the direction and close supervision of an RBT Supervisor and/or RBT Requirements Coordinator, who are responsible for all work the RBT performs.

Getting the RBT credential takes a 40-hour training, a competency assessment, an application and an exam. See the RBT pathway and the fast-track timeline.

Missouri note from our research: Missouri Medicaid allows a 90-day window from passing the initial competency assessment to hold the RBT credential, which lets new BTs start before exam results, but claims after day 90 without the credential are subject to recoupment (effective June 26, 2026 notice). Providers should track the exam date.
