Getting a new technician started in North Carolina
NC Medicaid gives new paraprofessionals 120 days from hire to obtain RBT (or QABA ABAT), so a BT can start under supervision while certifying; telehealth cannot be used for paraprofessional direct services. (Our research note; confirm with the agencies on the BT vs RBT page.)
What we are still checking
- Full text of Clinical Coverage Policy 8F (not opened; relied on NC Medicaid bulletins)
- Medicaid provider enrollment URL (NCTracks)
- NC licensure statute citation, requirements, fees, telehealth/telesupervision and BT rules of the Behavior Analyst Licensure Board
- Whether the autism mandate covers self-funded plans and current CPI-adjusted cap
- Commercial telehealth parity for ABA
- Major payers (Medicaid managed care plans, commercial)
- Background check requirements for BTs
- An advocacy-site article referenced further proposed policy changes with future dates; not relied upon
- NC licensure application requirements, fees, processing timelines and detailed supervision rules beyond the electronic-services position were not fully verified in this focused pass.
- Whether a particular ABA clinic or home-service agency requires Chapter 122C facility licensure depends on its operation and applicable exclusions; no blanket ABA-specific determination was verified.
- Background-check obligations for BTs outside settings covered by G.S. 122C-80 and outside the BACB RBT pathway were not comprehensively verified; QABA ABAT screening and training requirements remain unverified.
- The former practice_requirements assertion that Medicaid bulletins establish no paraprofessional background-check requirement was unsupported and has been replaced with setting-specific statutory and BACB requirements.
- Other substantive sections were preserved under the practice_requirements-only scope. Their statements that the licensure chapter and commercial/board BT rules were not verified are superseded by the Chapter 90, Article 43, G.S. 90-745 and Blue Cross NC findings recorded in practice_requirements.
- CCP 8F was opened in this pass. Attachment B initially measures certification timing from hire or the employing provider's first Medicaid enrollment, whichever is later, but later refers to cessation after 120 days from hire; the Aug 31 bulletin summarizes new hires as having 120 days from hire. Application of the later-enrollment provision requires payer clarification.
- Blue Cross NC's cited policy lists BCBAs among general ABT providers but its NC paraprofessional supervision clause refers only to listed professions 1-7, excluding the separately listed BCBA item 8. Whether an LBA-only supervisor satisfies that commercial clause was not resolved.
- Commercial BT credential and independent-billing requirements beyond the cited Blue Cross NC policy were not verified.
- Detailed approval procedures for Medicaid 97155 telehealth beyond the ordinary 50% limit were not verified; CCP 8F requires clinical justification in the treatment plan and does not establish automatic approval.
- medicaid_aba: verifier astra marked conflict - Coverage, agency identity, RBT/ABAT alternatives, August 1 policy update and supervision framework are supported. Enrollment has an unresolved source conflict: both bulletins specify August 2, 2026, but S.L. 2026-1 §3C.18(e) makes subsection (c) effective upon enactment; the law records approval April 30, 2026. The statute names BCBAs and Qualified Autism Services Practitioner Supervisors, whereas the bulletin addresses all LQASPs/C-QPs seeking enrollment. Report these separately. Do not imply every previously enrolled provider must reenroll. BCBA certification alone does not establish independent-practice authority. The 10% observation requirement specifically requires an LQASP. The service-hour ratio concerns each beneficiary's accumulated paraprofessional services, not one technician's total workload; CCP 8F uses more than 200 hours in 180 calendar days. The new-hire certification deadline also needs the later employer-enrollment qualification described in bt_vs_rbt. Incidental supervision/training is not independently reimbursable.
- bt_vs_rbt: verifier astra marked conflict - RBT or QABA ABAT certification, temporary LQASP-supervised delivery and nonreimbursement after the applicable grace period are supported. However, the new-hire deadline is incompletely stated: S.L. 2026-1 §3C.18(a)(8)(a) and CCP 8F Attachment B use the later of hire or the employing provider's initial Medicaid enrollment. The bulletin abbreviates this to hire; Attachment B also contains a subsequent cessation sentence referring only to hire, creating an internal drafting inconsistency. Preserve the bulletin's August 1 transition statement separately. Populate state_specific_bt_requirements with §§90-732(2) and 90-745(2): supervised implementation, supervisor responsibility, and no independent assessment/intervention-plan design. These statutory provisions do not impose a universal RBT credential. Medicaid reimbursement requirements must not be presented as a universal prohibition on uncertified technician work. Commercial-payer requirements remain unverified.
