ABA insurance and Medicaid coverage in Iowa
Last researched October 21, 2026
Who pays for ABA in Iowa, and what a practice has to do to bill them. Coverage details here come from statutes and agency pages we read, not from payer marketing.
Autism insurance mandate
- State mandate for autism coverage
Yes, the research found a state autism coverage law.
- Statute
Iowa Code 514C.31 (applied behavior analysis for treatment of autism spectrum disorder)
- Age limits
As of 2026-10-21, the age restriction (covered individuals under 19 years of age) appears to have been removed for covered policies delivered, issued for delivery, continued, or renewed on or after January 1, 2026 (HF330, 2025 Acts chapter 162). The 2025 Code text limited coverage to individuals under 19; that is historical for current guidance. The current 2026 coverage language refers to treatment 'provided by a practitioner to covered individuals' without an age limit.
- Annual cap
As of 2026-10-21, no ABA-specific annual maximum benefit appears to apply to covered policies delivered, issued for delivery, continued, or renewed on or after January 1, 2026: HF330 (2025 Acts chapter 162) struck the annual-benefit subsection (subsection 3 of the 2025 Code). Section 514C.22 additionally prohibits annual/lifetime limits and specified outpatient visit limits for autism spectrum disorder. Historical (2025 Code, now obsolete for current guidance): not less than $36,000 through age 6, $25,000 for ages 7-13, and $12,500 for ages 14-18, with payments for treatment other than ABA not counting toward the maximum.
- Excludes self-funded employer plans
We could not verify this yet - check with your state insurance department.
- Notes
Applies to group policies/plans issued by a carrier to an employer with more than 50 full-time equivalent employees (on at least 50% of working days in the preceding calendar year), and to chapter 509A public-employee plans other than State employees. The exclusion of individual accident and sickness policies issued to individuals, among other plan types, is now in subsection 8 (formerly cited as section 9). Prior authorization remains permitted; the visit-limit authorization for specified policies was removed by HF330 for policies delivered, issued for delivery, continued, or renewed on or after January 1, 2026. Treatment plan review is permitted no more than once every 3 months in year one and every 6 months thereafter, though more frequent review is permitted when the carrier and the treating physician or psychologist execute an individual-specific agreement. 'Practitioner' = licensed physician, licensed psychologist, or behavior analyst licensed under chapter 154D. The statute applies to policies delivered/issued/renewed on or after Jan 1, 2018 (2022 definitional amendments apply from Jan 1, 2023). Whether the mandate applies to self-funded (ERISA) plans is unresolved from this section alone; statutory silence does not establish an ERISA conclusion.
